First, on this form, check off the supply you would like.
Second, scroll down to the bottom of the form. If you have a Rapid Prototype request and have CAD drawings, please click on the e-mail link, attach the drawings, and send the e-mail.
Third, Click on the "Supply Request Requirements" link. A form will appear in a separate window. When you have filled out the Supply Requirements form with your specific requirements, submit the form.
Fourth, after sending the e-mail note and the Supply Requirement form, click on "Submit New Supply Request" and you will be done.
